ARE YOU NEW TO OUR PRACTICE?

If so, we again say welcome. As a new patient you can expect to fill out a new patient history. This information is necessary to provide you with the best dental care possible. Your first appointment will include a comprehensive oral examination and diagnostic x-rays. At this time, we will determine your needs for dental cleaning, examine your teeth for decay, diagnose any periodontal conditions that may exist and address any concerns you may have. After determining your needs we can provide a treatment plan to help you accomplish these recommendations.

Great! Please let us know if there have been any major changes in your life, such as change of dental insurance, address or health. Providing this information will help us serve you better.

 APPOINTMENTS

We feel that your time is valuable and we strive not to keep you waiting, in turn, we ask that you give us 24 hours notice to change any appointment that you may have. This will allow us to use this time to help others with their dental needs.
